[Dhsbb-jazz] Medication Assistance Authorization Form for Reno Jazz Festival

Hi Jazz Band Families,
    Because this is an overnight field trip, any medication (prescription
or over the counter) will be dispensed by Mr. Slabaugh on the trip.
   If your child will be needing any medication, please carefully read and
fill out this attached form and have it signed by a doctor. The signed form
and medications will need to be brought on Friday, April 26 when the
students depart.
    Though, if possible, please return the form by email to Lori Ferguson
at lori_ferguson at me.com before April 26, so all that needs to be brought on
the day of the trip will be the medications. This will help with the
organization of the paperwork ahead of time.
    All medications will need to be in their original packaging.
