Hello Parents,
I want to introduce myself--my name is Yvonne Otani and I'm a pediatrician at the Davis Kaiser Permanente.  I will be going to San Diego on the band field trip as the medical chaperone. District rules disallow me from checking the medical information in the district packets, and Mr. Slabaugh must be in charge of the medications. However, I will be available to your children during the entire trip.  I would be happy to speak to any parent prior to the trip about any medical issue concerning their child.
