Greeting Everyone,

Below is a link to our New DHS Bands Registration form.   It only takes a couple minutes and is CRUCIAL to getting the correct information out to everyone.
This is information that only the Band Booster Board and Mr. Slabaugh will have the ability to see.
Portions may be used for the Spring Trip or other events that require us to be able to contact parents, etc.

Please go to www.dhsbandboosters2.com<http://www.dhsbandboosters2.com> , and click on the “click here” link.
That will pull up the form for you to fill out.
Please note that fields with an “*” are required fields and must be filled out in order to submit the form.

Once you complete the student info, parent/guardian info, the lower section gives you the opportunity to check areas that you can help volunteer with.
You can check as many of these items as you like.

Once completed, please fill in the Captcha code and hit submit.  Wait a second, and you should get a screen back that shows your data for confirmation that it was accepted.
You should also get a response email from me, thanking you for taking the time to do this.
