[Dhsbb-jazz] Forms for Delta Jazz Festival

Jazz Band parents: Sharpen your pencil and fill out the required forms for the field trip to the Delta Jazz festival on Saturday, December 3.  All forms are on the band boosters website.  Please turn the forms in to Mr. Lange by next Wednesday, November 30.  Yes, you need to fill them out again even if you have done so earlier this year.


All students need forms 2a and 2b, parent/guardian school sponsored trip authorization.

Drivers need forms 3a and 3b, volunteer/employee use of auto statement and driver emergency procedure/insurance verification.  Attach a copy of the auto insurance card.  

And in a triumph of bureaucracy over common sense, parents driving their own child need form 5, parent/guardian request to transport own child.
