We will be driving our car and taking our son Jordan and band member Bobby Young.  I would like to leave from our home in South Davis rather than meeting all at the High School first.  I will submit forms to Fred by Thursday.

So far the list of drivers and number of students they are taking on Saturday to Stockton are:

Let me know if anyone else is driving.

 

Every student must fill out the attached form 2 with health insurance information.

 

All drivers must fill out the attached driver forms 3a and 3b and attach a copy of their auto insurance card.

 

And, from the department of redundancy department, the District requires parents who are transporting only their own child to fill out the attached form 5.

 

Please turn all forms in to Mr. Lange by Thursday.
