[Dhsbb-jazz] Folsom Jazz Festival Permission Slips

Hi All,
   
  Remember to complete and return  your student's permission slip for this Friday's jazz festival in Folsom.
   
  Please have your student give the signed form to Mr. Lange, OR...be sure your student has it this Friday in the Band Room at 5:00 PM.
   
  A copy of the form is attached.
   
  Also--if you are available to drive any students who need transportation, please fill out and bring the carpool form (also attached), along with proof of your automobile insurance.
